Pulmonary & Sleep Medicine Associates, LLP offers comprehensive care for lung and sleep conditions in the southern part of Atlanta. We were the first accredited sleep center in the area, and we're a highly experienced team that's been welcoming patients since 1986. We always offer personalized care to our patients at either our Stockbridge office or our Fayetteville office, and we're excited to welcome you today.

What to Bring to Your Appointment
- Photo ID
- Insurance card
- Physician referral forms (if required by insurance)
- List of current prescriptions and over-the-counter medications (including dosages and frequency)
- Pertinent medical and surgical history
- Any recent X-rays or appropriate records

Payment and Insurance Information
Pulmonary & Sleep Medicine Associates, LLP is a provider for Medicare and most major insurance plans. We provide insurance billing for your convenience.
Please be aware that any costs not covered by insurance are your responsibility. For services that are not covered by insurance, we request payment at the time of your appointment. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.
